HOW TO TREAT ULCER (STOMACH ULCER)


 Management of Gastritis and Peptic Ulcer Disease



The treatment of peptic ulcer and gastritis are similar, therefore, both will be discussed together in this article as one.

As I said under gastritis, this management is under two categories


-          Lifestyle control

-         Chemotherapy control


Lifestyles Control involves the modification of diet such as

Putting an end to alcohol intake and cigarette smoking

Reduction in usage of NSAIDs (Non-steroidalanti-inflammatory drugs) and other ulcer precipitating agents

Proper treatment of bacterial infection i.e. helicobacter pylori, a well known and established causative micro-organism implicated in ulcer

Note: From discussion with patients in a normal daily clinic, I have found the following implicated in reactivating and aggravating peptic ulcer.

These include the carbonated drinks(the so called soft drinks such as coke), excessive consumption of orange  especially the partially ripe ones and  consumption of frying foods such as plantain chips, roasted corn, including garri ( cassava flour) and  beans.

All these have been reported by patients to have set in a pepperish sensation after their consumption prior to their visitation to the clinic and a follow up of some of them has really shows good improvement in their health.

Report to hospital/your doctor for severe menstrual pain and low back pain that have not respond to low dose analgesic.


Chemotherapy

This is the use of medications either by injection or tablets. I will try to be more practical in this treatment, but I will advice you see your doctors or physicians if no improvement following the use of the combination below.

However, I can assure you that if you follow the lifestyle modification, especially those you can identify to reactivate, aggravate or can cause it and take the medication judiciously as I prescribed you will be sound and healthy
.
Outline as follow,

1. Menstrual pain start with Orphesic (Paracetamol plus Orphinadrine) ii tabs in the morning and night, if no improvement, Tab Diclofenac 50mg twice daily plus tab Omeprazole 20mg twice daily for 3 - 5 days. With this combination, you are protected from gastritis and peptic ulcers.

2. Mild to moderate low back and general body pain make use of Neurogesic Cream and 
 (Paracetamol plus Orphinadrine)  ii tabs twice daily or Diclofenac 50mg twice daily plus tab Omeprazole 20mg twice daily for 3 - 5 days.
However, due to many causes of low back pain and body pain, if there is no improvement, see your physician for better evaluation and management.


3. The drugs for ulcer are taken for 10 – 14 days on the average using the following combination

a. Tab Omeprazole 20mg twice daily  plus Tab Clarithromycin 500mg twice daily plus Tab Metronidazole( also known as  Flagyl )  400mg twice daily  all taken together for 10 -14 days.

b. Tab Omeprazole 20mg twice daily  plus Tab Clarithromycin 500mg twice daily plus Tab Amoxicillin(also known as Amoxyl) 500mg twice daily  all taken together for 10 -14 days.

c. Tab Omeprazole 40mg stat ( i.e. the first day),  follow by same Omeprazole 20mg twice daily  plus Tab Clarithromycin 500mg twice daily plus Tab Flagyl 400mg twice daily.  all taken together for 10 -14 days in situation where there is severe pain with or without passage of dark stool in the absence of iron containing drugs (blood tonic).

d. Tab Omeprazole 20mg twice daily plus Tab Amoxicillin 500 three daily plus Tab Flagyl 400mg three daily for 10 -14 days in mild ( especially if financially constraint).

e. Tab ULSAKIT, a single medication with combination of Clarythromycin, Omeprazole and Metronidazole in three different  coloured capsules. It is taken three twice daily till you exhaust the whole pack.

However, I still prefer to add other ulcer drugs in addition to all the combination above because a lot of patient will have use omeprazole before presentation and these are not the regular combination most especially in severe case.

These are Antacid ( Gascol, Relcer Gel, Gestid est.) 15mls 2-3 times daily or PRN after each of the combination plus Tab Cimetidine 400mg daily for 5- 7 days  but at night  after all medications and meals.
The presence of the following symptoms, severe epigastria (upper abdominal) pain , nausea and vomiting of blood, weight loss, passage of frank bloody stool/ dark stool ( indicate internal bleeding) and those resulting from complications, need a doctor evaluation and management in the hospital setting.





For those who prefer supplements for stomach ulcer, a combination of Gastrifort 
plus Constilease and Reisshi taking under appropriate prescription.







Finally, those drugs mentioned above are environment dependent  i.e. other brand maybe in use in your country, likewise, change in dosage may present and there are other management methods not discussed here, these are left for your physicians/doctors.
